emergency, please call our office as early in the day as
possible. After hours, our answering service will advise
you how to proceed.
Once you've made your appointment with us,
please print and fill out the required forms -
Medical History,
Registration Form, and
Insurance Disclaimer - and bring them with you to your
appointment.
